Returning Customer

Important Information for Returning Customers on Requesting an RA (Return Authorization)

A return authorization number is required for any devices/units which a new or returning customer would like to send in for repair.  In order to help with larger numbers of RA requests, we have provided this streamlined request form.

If you are a returning customer with an established
Credit Account (downloadable application form)  you may easily enter requests for one or more units by simply providing your customer number. Your customer number is available by request by email from our Administrative Desk.

Return shipping will be as on account.  If there are any changes to these arrangements please notify us in the additional shipping instruction box provided below.

You will receive an RA# by email and we will contact you if we require any additional information.
